Additional Signer form - this form allows you to add another authorized person to your ITEX account.  They will be allowed access to the account and have authorization to sign ITEX checks.

Sub Account Guidelines and Application- these forms will allow you to set up an employee account under your account so you can bonus your employees with ITEX.

Auto-pay Enrollment - this will allow you to change or update the EFT or checking account that is on file for your fees.

One Time EFT or CC payment - This form is for a one time payment for your fees that will not effect the auto-pay.
